Abstract
Two sprayings of 5% DDT in the Bas Zaire rural pilot project showed a 65.2% reduction in the slide positivity rate, reduced prevalence of the primary malarial vector Anopheles gambiae, and a lower fever case load at health centers. Achievements in urban areas were similar but were lessened by the refusal of many homeowners (33-47%) to have their homes sprayed. Additional rural entomological studies were conducted. The laboratory was reorganized; chiefs of laboratory and of entomology, a statistician, and six entomological aides have been hired; and staff were trained in various malaria detection/analysis techniques. Plans are underway to incorporate into the pilot project a limited antimalarial drug program. Implementation of a national malaria plan integrating malaria control into the overall health delivery system will be very difficult to achieve before project end.
