The HEW must tick all the relevant boxes to indicate the specific services for which the client is being referred. Upon receiving the referral, the health worker completes the counter-referral section of the form. This section includes the date, name of the facility, and name of the client. The health worker also provides remarks on the services provided, including any follow-up care that may be required. The form is then returned to the client, who shares it with the HEW.
