The USAID COVID-19 Vaccine IP Technical Assistance Forum conducted a pulse check in January 2022 to assess the challenges faced by countries in implementing COVID-19 vaccination programs.

Abstract
The survey received 21 responses from 10 projects and 7 countries, including Ethiopia, Botswana, Cameroon, Zambia, Ghana, India, and the Philippines. Health care worker capacity and fatigue were identified as significant challenges, with an average importance rating of 4.4 out of 5. Insufficient operational costs, vaccine confidence and low demand, regulatory authorization, and cold chain and storage requirements were also considered important challenges, with average importance ratings ranging from 4.2 to 3.7. Countries and projects are exploring various strategies to address these challenges. Communication and community-level demand creation efforts include continuously adapting and aligning with local priorities and approaches, requesting donor funding and private sector support, and working with Ministries of Health to ensure access to vaccines. Policy, planning, coordination, and cost-related challenges are being addressed through vaccine confidence mass and social media campaigns, faith leader engagement, mobilization, and interpersonal communication. Human resources and capacity building activities are also being implemented, including capacity building for health professionals and students, data generation and use, and RCCE training for health professionals and students. Interpersonal communication, community engagement, and social media campaigns are being used to address vaccine confidence and low demand. Continuous capacity building strategies, mobilizing multiple stakeholders and the private sector, and adapting to local contexts are also being employed. The lack of vaccines and uncertainty around future vaccine supplies were identified as significant challenges, with an average importance rating of 4.4 out of 5. Countries are addressing this challenge through microplanning, vaccine confidence mass and social media campaigns, faith leader engagement, mobilization, and interpersonal communication. Regulatory authorization is also a challenge, with countries working to ensure access to vaccines by obtaining regulatory approval. Cold chain and storage requirements are a significant challenge, particularly in hot countries with poor infrastructure. Countries are addressing this challenge through the use of ultra-cold chain equipment and private sector support. Vaccine confidence and low demand are also significant challenges, with countries using various strategies to address these issues, including interpersonal communication, community engagement, and social media campaigns. The survey highlights the importance of addressing these challenges to ensure the successful implementation of COVID-19 vaccination programs. Countries and projects are exploring various strategies to address these challenges, including communication and community-level demand creation efforts, policy, planning, coordination, and cost-related challenges, human resources and capacity building activities, and adapting to local contexts.
