Facility FP/MIYCN Integration Supplementary Register is used to document additional information generated as part of Family Planning (FP) and Maternal and Infant Young Child Nutrition (MIYCN) integration.

Abstract
The register is not intended to replace existing records. It is essential to fill out the register as completely as possible, following the provided instructions. For each space in the register, either tick where applicable or follow other instructions where provided. If the answer is no, mark with an "X." If the column does not apply to the household member, mark with a dash (-). A new register sheet should be started at the beginning of each month. The remarks column should be used to note comments only as they relate to FP/MIYCN, for the benefit of the service provider. The register includes various sections, such as client information, caregiver condition, infant feeding, underweight infant, current FP method, counseling, medicines administered, referrals, and remarks. Client information includes date, client number, client name, client sex, and caregiver condition. Caregiver condition includes pregnant and postnatal status, with specific details on infant age. Infant feeding practices are documented, including exclusive breastfeeding (EBF), complementary feeding (CF), mixed feeding (MF), and replacement feeding (RF). The current FP method is recorded, including Lactational Amenorrhea Method (LAM), transitioning from LAM to other methods, and other family planning methods. Counseling on FP, maternal nutrition, LAM, and infant and young child nutrition (IYCN) is also documented. Medicines administered include deworming, iron-folate, and Intermittent Presumptive Treatment (IPTp2+). Maternal mid-upper arm circumference (MUAC), maternal body mass index (BMI), and hemoglobin level from the ANC profile are recorded. Referrals to other service areas are documented, including the reason for referral. The register also includes spaces for page summaries and monthly summaries at the bottom of each page and the last sheet used in the month, respectively. Total referrals IN and OUT are recorded, counting only those referred for FP, Nutrition, Danger Signs, Home Delivery, and Postnatal Visit.
