Abstract
More than 80% of individuals in Zambia have a close relative who has died of AIDS, highlighting the need for care and prevention. A continuum of care with links and coordination between clinical services and community and home services is essential for the patient and the nation. Experience shows that in a community where needs are great, resources are few, and potential stigma of people living with HIV/AIDS (PLWHA) is great, home-based care is very effective in helping families cope. Studies also demonstrate that cost-effective home-based care can be implemented with minimal resources. The Continuum of Care model involves different levels of care, from medical care at a hospital to support from the community and basic care in the home. This model is best for the patient and the nation, as it ensures that individuals receive the necessary care and support at each stage of their illness. Home-based care is particularly effective in communities where resources are limited and stigma is high. This approach allows families to provide care and support to their loved ones in the comfort of their own homes, reducing the need for hospitalization and other costly interventions. The Continuum of Care model also emphasizes the importance of coordination and linkages between clinical services and community and home services. This ensures that individuals receive comprehensive care and support, from diagnosis to end-of-life care. In Zambia, the Continuum of Care model has been implemented in various communities, with positive results. Studies have shown that home-based care can be effective in reducing hospitalization rates, improving quality of life, and reducing healthcare costs. The model also highlights the importance of community involvement and participation in care and support. Community-based care workers, trained to provide basic care and support, play a critical role in implementing the Continuum of Care model. In addition, the model emphasizes the need for education and awareness about HIV/AIDS, as well as the importance of reducing stigma and discrimination against PLWHA. By addressing these issues, communities can create an environment that supports and empowers individuals and families affected by HIV/AIDS. The Continuum of Care model is a comprehensive approach to addressing the needs of individuals and families affected by HIV/AIDS in Zambia.
