Abstract
The data collection form includes various fields such as the number of the file, the name of the mother, the date of birth, the date of the last menstrual period or gestational age, the weight of the newborn, the sex of the newborn, the type of delivery, and the respiratory status at birth. The respiratory status at birth is categorized into two types: 1) crying/respiring and 2) not crying/not respiring. The form also inquires about the resuscitation performed in the first minute, which includes five possible interventions: 1) no resuscitation, 2) drying and stimulation, 3) aspiration of secretions, 4) ventilation with mask and bag, and 5) intubation. The form asks whether the newborn was breathing after resuscitation and categorizes the outcome into two types: 1) alive and 2) deceased. The form also collects information on the temperature of the newborn, which includes four possible scenarios: 1) the newborn was dried immediately and wrapped, 2) the newborn was dried immediately and placed in skin-to-skin contact with the mother, 3) the newborn was dried and placed under a heat lamp, and 4) no intervention was performed. Additionally, the form inquires about the care of the umbilical cord, which includes three possible scenarios: 1) the cord was cut with a sterile instrument (autoclave or bistoury), 2) the cord was cut with an episiotomy instrument, and 3) an antiseptic was applied (specify). The form also collects information on prophylaxis ocular, which includes two possible scenarios: 1) yes and 2) no. Furthermore, the form inquires about exclusive breastfeeding, which includes two possible scenarios: 1) breastfeeding was initiated in the first hour and 2) breastfeeding was initiated later. The form is designed to collect comprehensive data on newborns, which can be used for various purposes such as research, quality improvement, and public health initiatives.
