Additionally, the form inquires about the care of the umbilical cord, which includes three possible scenarios: 1) the cord was cut with a sterile instrument (autoclave or bistoury), 2) the cord was cut with an episiotomy instrument, and 3) an antiseptic was applied (specify). The form also collects information on prophylaxis ocular, which includes two possible scenarios: 1) yes and 2) no. Furthermore, the form inquires about exclusive breastfeeding, which includes two possible scenarios: 1) breastfeeding was initiated in the first hour and 2) breastfeeding was initiated later. The form is designed to collect comprehensive data on newborns, which can be used for various purposes such as research, quality improvement, and public health initiatives.
