Abstract
The document is a monthly QA report for the project WO-LT-0082 Amendment 3, which is the American University 200-Bed Women's Dormitory. The report covers the period from June 4 through July 1, 2016. Main construction activities during this reporting period included excavation, backfilling, compaction, and testing of sewer utility trenches, leak testing of installed sewer utilities, excavation, backfilling, compaction, and testing of electrical utility trenches, water well drilling and installation, placement and curing of concrete manhole bases, and replacement of unapproved backfill material in utility trenches with approved fill. Construction issues impacting progress included the re-alignment of the electrical utility, use of unapproved backfill for sewer utility, placement of concrete prior to field density testing on the underlying base of manholes, and working without pre-approved plans. The contractor has acknowledged working at his own risk and has requested approval of required pre-construction plans, including health and safety, quality control, environmental protection, security, and implementation plans. The contractor has made progress on various activities, with the following percentages completed: CIV310 sewer system trench/manhole excavation (97%), CIV320 sewer system manhole base preparation/trench sand bed (90%), CIV330 sewer system piping placement (92%), CIV350 sewer system pipe leakage test (90%), CIV360 sewer system trench/manhole backfilling/sieved soil placement over pipes (19%), CIV340 sewer system precast manhole placement (22%), and CON100 site preparation, grubbing, and leveling (20%). Meetings were held during this reporting period, including a meeting to introduce Tetra Tech QA inspectors to the contractor staff, four weekly coordination/progress meetings, and a preparatory meeting to discuss upcoming phases of work for sewer and electrical manholes installation. Design and drawing review submittals were received from the contractor, with 21 (30%) approved as submitted, 40 (59%) rejected and requiring re-submittal, and seven under review with Tetra Tech. The average length of time for Tetra Tech to process a submittal was 9.34 days, and the average length of time for the contractor to return a rejected submittal was 19.32 days. Field observations and site visit details were conducted, with the status of significant construction activities provided in a table. The table shows the start and finish dates for various activities, as well as the percentage of work completed and the deviation from the approved baseline plan/schedule.
