Abstract
This job aid is intended for providers initiating a client on PrEP or helping a client who is struggling to use PrEP as prescribed. The job aid begins by sharing any limits to confidentiality and administering a standard set of questions to identify clients experiencing intimate partner violence (IPV). If the client answers yes to any of the questions, the provider decides whether to tell the partner about PrEP use. If the client does not want to tell the partner, the provider informs the client about violence response resources available and discusses strategies for taking PrEP without the partner's knowledge. The job aid also provides tips for telling a partner about PrEP use, including how to have the conversation, where to have it, and when to have it. If the client wants the provider's help telling the partner about PrEP use, the provider schedules an appointment for the client and partner. In cases where the client has experienced IPV, the provider responds to the disclosed violence using the LIVES approach: Listen, Inquire, Validate, Enhance Safety, and Support. The provider also counsels the client on PrEP use within abusive and controlling relationships, emphasizing the importance of PrEP in reducing vulnerability to HIV. The job aid also addresses the decision of whether to tell the partner about PrEP use, taking into account the client's concerns about abusive behavior by the partner. The provider asks the client how they would like to proceed and offers support and resources as needed. The job aid is designed to be adaptable to local specifications and clinic processes, and it includes a detailed flow chart with suggested scripts and monitoring processes. The job aid is intended to be used in conjunction with the standard operating procedure and is not a standalone document. The PrEP Job Aid is the result of a collaboration between the Community Health Clinic Model for Agency in Relationships and Safer Microbicide Adherence (CHARISMA) project, the Meeting Targets and Maintaining Epidemic Control (EpiC) project, the Reaching Impact, Saturation, and Epidemic Control (RISE) project, and the Collaboration for HIV Prevention Options to Control the Epidemic (CHOICE) activity, with funding from the American people through the United States Agency for International Development (USAID) and the U.S. President's Emergency Plan for AIDS Relief (PEPFAR). The job aid is designed to support providers in addressing partner relationships and IPV in PrEP services, and it provides a framework for discussing PrEP use with clients who have experienced IPV. The job aid emphasizes the importance of confidentiality, safety, and support in PrEP services, and it provides resources and guidance for providers working with clients who have experienced IPV.
