The course of her labor and delivery should be reviewed to ensure that none of the following conditions are present: chorioamnionitis during labor, more than 18 hours from rupture of membranes to delivery of baby, unresolved postpartum hemorrhage, puerperal sepsis, postpartum endometritis/metritis, continued excessive postpartum bleeding, or extensive genital trauma where the repair would be disrupted by immediate postpartum placement of an IUCD. Availability of sterile instruments and IUCDs on the labor ward should also be confirmed. If any of the conditions listed are present, insertion of the IUCD should be deferred, and the woman should be provided with information about alternative methods. If all conditions are met, insertion of the IUCD can proceed. Immediate postpartum insertion of the IUCD is a safe and effective method for family planning. However, it requires careful consideration of the woman's clinical situation and the availability of necessary resources. The procedure should only proceed if the woman's antenatal record confirms that an IUCD is an appropriate method for her, and if none of the listed conditions are present. Preparation for IUCD insertion involves confirming the availability of sterile instruments and IUCDs on the labor ward. If these resources are not available, insertion can proceed if they become available within an appropriate time frame.
