Supportive Supervision Checklists for District Health Services (DHS) and Health Facilities are designed to monitor and evaluate the performance of immunization programs.

Abstract
The checklists assess various aspects of cold chain management, quality and logistics, monitoring and surveillance, program management, and injection safety. Cold Chain, Quality and Logistics The checklists evaluate the functioning of refrigerators, temperature recording, and vaccine storage. They assess whether vaccines and supplies are loaded properly, whether the refrigerator is functioning for the entire month, and whether there are adequate stocks of vaccines and supplies for Routine Immunization (RI). The checklists also evaluate the availability of safety boxes and the recording of vaccine distribution and arrival. Monitoring and Surveillance The checklists assess the monitoring and surveillance activities of DHS and health facilities. They evaluate the availability of up-to-date monitoring charts, the drop-out rate of Penta, and the reporting of Vaccine-Preventable Disease (VPD) cases. The checklists also assess the tracking of defaulters and the availability of a system for tracking defaulters. Programme Management The checklists evaluate the program management activities of DHS and health facilities. They assess the availability of quarterly micro-plans, the implementation of immunization services according to the Basic Service Package (BSP), and the receipt of supervision visits. The checklists also evaluate the reporting of immunization sessions and the availability of a system for tracking defaulters. Advocacy and Communication The checklists assess the advocacy and communication activities of DHS and health facilities. They evaluate the availability of Immunization, Education, and Communication (IEC) materials, the participation of health staff and community in review and micro-planning meetings, and the organization of meetings with health staff and partners. Injection Safety The checklists evaluate the injection safety practices of health facilities. They assess the availability of functional incinerators and the safe disposal of waste. The checklists are designed to be used by supervisors to monitor and evaluate the performance of DHS and health facilities. They provide a framework for identifying areas of improvement and for developing action plans to address these areas. The checklists are used to identify main problems and to develop action plans for follow-up.
