Abstract
The plan aims to guide the assessment of progress toward stated results and provide detail regarding planned activities for the subsequent project year. The FHN activity is a five-year project, spanning from June 28, 2021, to June 27, 2026, with a goal of contributing to a reduction in the maternal mortality rate (MMR) and under-five mortality rate (U5MR) in Zambia. The project's objective is to build capacity of the Government of the Republic of Zambia (GRZ), communities, and local partners to improve health system accountability and performance to deliver high-quality, accessible Reproductive, Maternal, Newborn, Child, and Adolescent Health and Nutrition (RMNCAH&N) care. The FHN activity will achieve its objectives through three key areas: improved Ministry of Health (MoH) capacity to deliver high-quality health services at subnational levels, strengthened health systems at national and subnational levels to create an enabling environment for effective RMNCAH&N service delivery, and increased community engagement in health service delivery. The plan outlines the performance management framework, which includes the Theory of Change and Results Framework, indicators, data management, and data quality management. The FHN activity will use the Health Management Information System (HMIS) and the National AV (NAV) - Enterprise Resource Planning Software to collect and manage data. The plan also emphasizes the importance of data quality management, including data quality assessments and critical assumptions and risks. The FHN activity will also implement a Collaborating, Learning, and Adapting (CLA) approach, which includes a learning agenda, illustrative FHN activity learning questions, and data use. The plan outlines the roles and responsibilities of the FHN activity team, including the Chief of Party, implementing partners, and other stakeholders. The FHN activity will be evaluated annually, and the plan will be reviewed and revised as needed. The plan also includes annexes that provide additional information on performance indicators, data quality standards, and compliance with USAID policies.
