The Community Health Worker (CHW) Family Planning/ Maternal, Infant, and Young Child Nutrition (FP/MIYCN) Integration Supplementary Register is a tool used to document additional information generated as part of FP/MIYCN integration.

Abstract
The register is designed to be used in conjunction with existing registers and is intended to be completed as thoroughly as possible. The register is divided into several sections, including household information, client information, and FP/MIYCN data. Household information includes the household code, individual code, and names of household members. Client information includes the date of visit, sex of household member, caregiver condition, and infant feeding practices. Caregiver condition is categorized into pregnant, postnatal, or neither. Pregnant clients are those who are pregnant or in labor, while postnatal clients are those who have completed the third stage of labor and have a child less than or over six months old. Infant feeding practices are categorized into exclusive breastfeeding (EBF), complementary feeding (CF), mixed feeding (MF), or replacement feeding (RF). FP/MIYCN data includes information on the mother's participation in breastfeeding and ANC support groups, current family planning method, and counseling provided. Family planning methods include LAM, transitioning from LAM, and other methods such as NFP, Condom, COC, POP, DMPA, Implant, IUCD, TBL, and Vasectomy. Counseling on FP/MIYCN is also documented. Referral information is also recorded, including whether a referral was provided, the reason for the referral, and the type of referral (FP, N, or O). Remarks are used to record additional comments related to FP/MIYCN to aid in follow-up. The register is designed to be used on a monthly basis, with a new sheet started at the beginning of each month. Page summaries are recorded at the bottom of each page, and monthly summaries are recorded at the bottom of the last sheet used in the month. Total referrals provided are only counted for family planning (FP) or nutrition (N) referrals. The CHW uses the supplementary register to document additional information generated as part of FP/MIYCN integration, which is used to inform follow-up and improve the quality of services provided to clients.
