CHW FP/MIYCN Integration Supplementary Register Instructions and Definitions

The register is designed to be used in conjunction with existing registers and is intended to be completed as thoroughly as possible. The register is divided into several sections, including household information, caregiver condition, infant feeding practices, FP method, counseling, referrals, and remarks. Household information includes the household code, individual code, name of household member, sex of household member, and date of visit. Caregiver condition includes information on pregnancy, postnatal status, and infant feeding practices. Infant feeding practices are categorized into four types: exclusive breastfeeding (EBF), complementary feeding (CF), mixed feeding (MF), and replacement feeding (RF). The register also includes information on whether the mother is a member of a breastfeeding and/or ANC support group. The FP method section includes information on whether the mother is using the Lactational Amenorrhea Method (LAM), transitioning from LAM to another method, or continuing to use another FP method. The register uses specific codes to identify different FP methods, including NFP, Condom, COC, POP, DMPA, Implant, IUCD, TBL, and Vasectomy. Counseling on FP/MIYCN is documented by ticking a box, and referrals are also recorded by ticking a box. The reason for the referral is specified as either FP, N, or O (other). The remarks section is used to record additional comments related to FP/MIYCN to help the CHW with follow-up. The register is designed to be used on a monthly basis, with a new sheet started at the beginning of each month. Page summaries are recorded at the bottom of each page, and monthly summaries are recorded at the bottom of the last sheet used in the month. Only referrals provided for FP or N are counted in the total referrals provided.
