The STOP Malaria Campaign household questionnaire is a tool used to gather information about household members' access to and use of insecticide-treated nets (ITNs), antenatal care (ANC), and malaria testing and treatment.

Abstract
The questionnaire is designed to be administered to the head of the household or an adult member of the household. The questionnaire begins by collecting demographic information about the household, including the number of households visited, the number of people who slept in the household the previous night, and the number of children under the age of 5 who slept in the household. It also asks about the number of pregnant women who slept in the household. Section 2 of the questionnaire focuses on ITN ownership and use. It asks about the number of sleeping spaces in the household, whether the household has any mosquito nets that can be used while sleeping, and how many mosquito nets the household has. It also asks about the number of sleeping spaces that have mosquito nets and whether the household has enough mosquito nets to cover all sleeping spaces. Section 3 of the questionnaire is dedicated to pregnant women. It asks about the number of months pregnant the women are, where they expected to attend antenatal care last month, and whether they received any antenatal care at a health center in the past month. It also asks about the expected and actual receipt of intermittent preventive treatment of malaria in pregnancy (IPTp) and whether the women received IPTp last month. Section 4 of the questionnaire focuses on counseling. It asks the respondent to tick in each box after providing counseling on ITN ownership and use, ANC attendance and IPTp uptake, and malaria testing and treatment. The questionnaire also includes a section for recording the total number of household members who slept under an ITN last night, the total number of children under the age of 5 who slept under an ITN last night, and the total number of pregnant women who slept under an ITN last night. It also asks about the number of household members who were ill with a fever in the last 2 weeks, the number of household members who got a test for malaria, and the number of household members who tested positive for malaria. The STOP Malaria Campaign household questionnaire is an essential tool for gathering data on household members' access to and use of ITNs, ANC, and malaria testing and treatment. The data collected through this questionnaire can be used to inform program implementation and evaluation, and to improve the effectiveness of malaria control efforts.
