Abstract
The initiative began in 2018 with a focus on improving blood pressure management among adults. Initial assessments revealed that nearly 30% of the population in the Western Province had hypertension, with a significant proportion of individuals unaware of their condition. The project implemented a multifaceted approach to address hypertension, including community-based health education, training for healthcare workers, and the establishment of blood pressure monitoring stations. Community health workers were trained to educate individuals on the risks associated with hypertension and the importance of regular blood pressure checks. Additionally, healthcare workers received training on the proper diagnosis and management of hypertension. A key component of the project was the establishment of a network of community-based blood pressure monitoring stations. These stations were equipped with automated blood pressure machines and were staffed by trained community health workers. The stations provided a convenient and accessible means for individuals to have their blood pressure checked, and the data collected was used to inform public health interventions. The project also focused on improving access to antihypertensive medications. A system was established to ensure that individuals with hypertension had access to affordable medications, and healthcare workers were trained to provide counseling on medication adherence. The project also worked with local pharmacies to ensure that they had a sufficient stock of antihypertensive medications. The project's geographic focus was on the Western Province, where hypertension was identified as a significant public health concern. The project's timeframes were divided into two phases, with the first phase focusing on community-based health education and training for healthcare workers, and the second phase focusing on the establishment of blood pressure monitoring stations and the improvement of access to antihypertensive medications.
