Abstract
The test is designed to improve TB case detection, particularly for smear-negative disease associated with HIV, and to expand capacity to diagnose MDR-TB. MDR-TB poses significant challenges due to its complex diagnostic and treatment requirements, while HIV-associated TB often goes undetected due to limitations in current diagnostic techniques. The global emergence of extensively drug-resistant TB (XDR-TB) and documented institutional transmission have highlighted the need for rapid diagnostic methods. The Xpert MTB/RIF test is a genotypic (molecular) method that offers several advantages, including speed of diagnosis, standardized testing, potential for high throughput, and fewer requirements for laboratory biosafety. The test has been endorsed by the World Health Organization (WHO) as a commercially available liquid culture system and molecular line probe assay for rapid detection of MDR-TB. Implementation of the Xpert MTB/RIF test requires careful consideration of several factors, including positioning of the test and site selection criteria, selection of individuals to test and patient management, and interim case definitions and patient registration. The test is particularly useful for diagnosing TB in patients with HIV, as it can detect TB in individuals with low CD4 counts.
