The Afya Ugavi Activity has been implementing health supply chain strengthening activities in collaboration with the Government of Kenya (GoK)/ Ministry of Health (MOH) at the national level and with select county government departments of health since October 2016.

Abstract
The Activity will end in December 2021 after completion of its 5-year term. Implemented activities currently focus on strengthening the supply chain for HIV/AIDS, Malaria, Family Planning/Reproductive Health, and Maternal, Newborn Child and Adolescent Health (FP/RMNCAH) both at the national and in all the 47 counties. This is done with a diffusive effect on the entire health supply chain, including essential medicines and medical supplies (EMMS). Per modification #19 of 2020, Afya Ugavi is currently supporting integrated supply chain activities in 36 counties, while coordinating Health Products and Technologies (HPTU) establishment countrywide in all 47 counties. Under the Malaria Program, Afya Ugavi supported the Division of National Malaria Program (DNMP) to carry out quantification for malaria commodities for the period 2022-2025. The Activity also supported development of the malaria commodities specification catalogue to guide procurement of malaria commodities by all funding agencies and minimize litigation arising from procurement. AU supported DNMP to review malaria commodities stock status. Support to the counties included supportive supervision, supply chain audits, commodity data verification and redistribution. Commodities worth $86,171 were redistributed during the year to avert expiries and mitigate stock outs. As of August 2021, coverage for long lasting insecticidal nets (LLINs) was 54% and 53% for antenatal care and child welfare clinics clients, respectively. Under the Family Planning/ Reproductive, Maternal, Neonatal, Child and Adolescent Health (FP/RMNCAH) program area, the Activity gave technical assistance towards the FP Logistics Working Group to hold two meetings, where an allocation of Kshs. 800m by MoH was announced during the world contraceptive day celebration; this pledge was being followed up by the FP program manager, which may ensure donor funding under the matching fund agreement. USAID also confirmed procurement of 320,400 pieces of 1 rod, while at the same time the Activity pledged to support FP program staff in any supervisions they chose. Under the HIV program, AU supported several meetings to deliberate on progress made nationally in the transition of patients from Tenofovir/Lamivudine/Efavirenz (TLE) to Tenofovir/Lamivudine/Dolutegravir (TLD) and to other antiretroviral therapy (ART) regimens. It was observed that TLE was generally overstocked at county level. AU also supported the annual forecasting and supply planning through technical and logistical support. For instance, Kakamega County conducted an annual quantification which included Kshs. 474 million for ARVs and HIV laboratory items that constituted 45% of the national level commodity support required by the county. In the three focus counties for FY20, reporting rates and timeliness of reporting averaged 98% compared to a national average of 90% for the same. Further, the Activity supported the National AIDS and STI Control Programme (NASCOP) to review the HIV Commodities Dashboard on KHIS - still at testing stage to check correctness and suitability of outputs from the dashboard. At the national Ministry of Health (MOH) level, Afya Ugavi supported the Division of Health Products and Technologies (DHPT) in the finalization of the Health Products and Technologies (HPT) supply chain strategy signoff by the ministry leadership, organizational development (OD) technical assistance and review of HPT guidelines. Development of data visibility dashboards continued, with the rollout of MoH 647 tool for EMMS monitoring and reporting at facility level, along with the necessary foundation for data absorption and visualization through KHIS. In FY21, the Afya Ugavi Activity was tasked by USAID to lend technical support to MEDS (Mission for Essential Drugs and Supplies) in readiness for its role as a possible recipient and distributor of USAID funded health program commodities. MEDS has since been contracted by the mission for this role and is now implementing the earlier anticipated role. Afya Ugavi supported MEDS in the review of its procurement and inventory management policy and procedures with the aim of aligning with best practices. In support of the order receipt process, Afya Ugavi supported the development of a tool that enables batch order uptake straight into the Enterprise Resource Planning software (ERP) for processing, eliminating human intervention. The program converts the order into a file format that is readily acceptable. In addition, the Activity is supporting the development of an online platform that would receive orders directly submitted by the counties. As part of support for warehouse optimization, Afya Ugavi supported the implementation of an Activity-Based Cost model that continuously measures performance and cost aimed at continuous improvement.
